General Terms and Conditions:
- Your card has been pre-loaded with $1000 in savings and may be redeemed at the office of Dr. Ken Williams and the Irvine Institute of Medicine and Cosmetic Surgery.
- For services such as Botox, dermal fillers and skin care: Present your card before your purchase or service, and we will deduct 15% off the total retail value.
- Card does not apply toward insurance-related cases, discounted procedures, operating room or anesthesia costs.
- All discounts will be deducted from the balance of your card. You may use your card as often as you like until your balance is $0, or until your card expires.
- One card per transaction.
- Your card is transferable to family & friends.
- Your card may not be valid with other discount offers or advertised specials. Restrictions apply to currently book surgery.
- Your card is not valid toward prior purchases, previous purchased services associated with other discounts or coupons.
- Your card excludes state & local taxes.
